Job Title: Lead Patient Care Coordinator

Providence Health & Services is seeking a highly skilled and experienced Lead Patient Care Coordinator to join our team. As a key member of our clinical operations team, you will be responsible for coordinating seamless care for patients, balancing complex daily workloads, and providing exceptional customer service.

Key Responsibilities:
  • Coordinate patient care activities, ensuring timely and effective delivery of services.
  • Develop and implement processes to improve patient flow and reduce wait times.
  • Collaborate with healthcare teams to ensure seamless transitions of care.
  • Provide coaching and guidance to Patient Care Coordinators, promoting professional growth and development.
  • Participate in quality improvement initiatives, identifying areas for improvement and implementing changes.
Requirements:
  • 3+ years of experience in a Patient Care Coordinator or customer service role, with progressively increasing responsibilities.
  • 2+ years of experience in an administrative medical office role, preferably in Providence or a similar healthcare organization.
Preferred Qualifications:
  • 2+ years of Electronic Medical Records (EMR) experience.
  • 6+ months of progressively responsible leadership experience.
