Job Summary:
The Case Manager, Galvan Homeless Shelter is a critical role in our organization, responsible for assisting families and individuals who are homeless or at risk of homelessness. This position requires a compassionate and dedicated individual who can work closely with clients to develop and implement personalized service plans, connect them with community resources, and advocate on their behalf.

Key Responsibilities:
Conduct comprehensive assessments of clients' needs and develop individualized service plans to address their unique challenges.
Coordinate with community resources, such as housing providers, healthcare services, and social services, to ensure clients receive the support they need.
Act as an advocate for clients, navigating complex systems and advocating on their behalf to ensure their rights are respected.
Provide ongoing support and guidance to clients, helping them to achieve their goals and maintain stability in their lives.
Collaborate with colleagues and community partners to ensure a comprehensive and coordinated approach to serving clients.
Document all interactions with clients, including assessments, service plans, and progress updates, to ensure accurate and timely reporting.

Requirements:
Associate's degree required; Bachelor's degree in a human services field preferred.
At least two years of experience in case management or a related field.
Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
Ability to work independently and as part of a team, with a strong commitment to collaboration and coordination.
Valid New York State driver's license and ability to meet agency driver requirements.
Familiarity with public assistance programs and community resources.
Experience with MS Word and Excel, with proficiency in Google Docs a plus.
Ability to pass required pre-employment background checks, including fingerprinting.
