Job Summary

The Case Manager, Galvan Homeless Shelter is responsible for assisting families and individuals who are referred to the Columbia County Homeless Family Program in maintaining their current housing or finding new and permanent housing.

The Case Manager performs an assessment of the client's service needs and develops a Service Plan to lead to a permanent residence, stabilization, and long-term amelioration of the issues that led to homelessness.

Key Responsibilities
  • Conduct intake and assessment process, develop and coordinate service plan delivery, and document all service delivery activities.
  • Meet with families to assist with service plan goals, follow up with families and service providers, and communicate information in a professionally responsible manner.
  • Engage with families to assure a collaborative and mutually agreed-on service plan, conduct regular service plan reviews, and maintain after-care contacts.
  • Establish and maintain professional working relationships with families, collaborators, and resource people, and act as an advocate for homeless families in the community.
  • Assist families in obtaining necessary services to retain permanent housing, identify and resolve barriers to housing retention, and provide intensive case management services in areas such as self-sufficiency, home maintenance, budgeting, and parenting skills.
Requirements
  • Associates degree required; BSW or Bachelor's Degree in human service field preferred.
  • At least two years of experience in case planning or case management.
  • Professional record of integrity and strong advocacy for homeless families.
  • Valid New York State Driver's License and ability to meet criteria as outlined in the Policy for Agency Drivers.
  • Familiarity with public assistance and other local resources.
  • Experience with MS Word required; Excel and Google Docs preferred.
