Key Responsibilities:
- 1. Medication Distribution: Address and resolve medication distribution challenges, ensuring precise and prompt delivery of medications to patients.
- 1.1 Manage incoming calls regarding medication requests, providing appropriate responses or directing inquiries to the relevant personnel.
- 2. Clinical Data Interpretation: Analyze clinical and pharmacokinetic laboratory results to develop effective medication monitoring strategies.
- 2.1 Demonstrate a solid understanding of fundamental pharmacokinetic concepts.
- 3. Problem Solving: Identify and address issues effectively.
- 3.1 Communicate and document findings, recommendations, and necessary follow-up actions.
- 4. Drug Information Provision: Offer comprehensive drug information to healthcare professionals, including nurses and physicians.
- 4.1 Act as the primary resource for drug-related inquiries from medical staff.
- 5. Educational Contributions: Engage in and contribute to educational initiatives within the department, both written and verbal.
- 5.1 Participate actively in departmental educational forums.
- 6. Documentation of Variances: Record medication use variances (MUVs) and adverse drug reactions (ADRs).
- 6.1 Propose actionable solutions to mitigate the recurrence of MUVs.
- 7. Compliance with Regulatory Standards: Uphold principles and conduct consistent with a High Reliability Organization.
- 7.1 Demonstrate effective compliance with safety and quality regulations.
Qualifications:
Education: A degree from an accredited College of Pharmacy (B.S. or Pharm.D.) is required. Completion of a residency in hospital pharmacy from an accredited program is preferred but not mandatory.
Experience: A minimum of two years of experience in an acute care hospital pharmacy setting is preferred.
Licensure: Must be licensed to practice pharmacy in Connecticut or eligible for reciprocity.
Special Skills: Candidates should possess a strong foundation in pharmacology and therapeutics, along with a thorough understanding of drug distribution systems and formulary management. Excellent communication skills and the ability to collaborate effectively with others are essential.
