Job Title: Home Health Registered Nurse RN Full Time

Job Summary:

Aveanna Healthcare is seeking a skilled and compassionate Home Health Registered Nurse RN to join our team. As a Home Health Registered Nurse RN, you will provide high-quality patient care in the comfort of their own homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities:

  • Develop and implement comprehensive care plans in collaboration with patients, families, and healthcare teams.
  • Provide skilled nursing care, including wound care, infusion therapy, and catheter management.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address them.
  • Collaborate with primary caregivers to ensure optimal patient outcomes.
  • Document patient progress, identify areas for improvement, and adjust care plans accordingly.

Requirements:

  • Active RN license in the state of application.
  • Valid CPR certification.
  • 1-year RN experience in a healthcare setting, preferably in home health or skilled nursing.

Preferred Qualifications:

  • Medicare skilled nursing experience.
  • Basic understanding of Oasis.
