Job Summary:

We are looking for a compassionate and dedicated Registered Nurse to join our team. As a Registered Nurse Home Health Care, you will be responsible for providing personalized care to our clients in the comfort of their own homes. Your primary focus will be on delivering exceptional patient care, promoting independence, and enhancing the overall quality of life for our clients.

Responsibilities:
  • Provide skilled nursing care to clients in their homes, including administration of medication, wound care, and treatments.
  • Develop and implement individualized care plans in collaboration with clients, families, and healthcare teams.
  • Conduct thorough assessments, evaluations, and interventions to ensure optimal patient outcomes.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
  • Document all patient interactions, interventions, and outcomes accurately and thoroughly.
Requirements:
  • Graduation from an accredited nursing program.
  • Current licensure as a Registered Nurse in the state of Delaware.
  • Experience in home health care or a related field preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
