Responsibilities:
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Provide skilled nursing care, including wound care, infusions, catheter care, and PICC line dressing changes.
- Conduct thorough assessments, identify patient needs, and develop strategies to address them.
- Collaborate with healthcare teams to ensure seamless care transitions and coordination.
- Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques.
- Document patient progress, identify areas for improvement, and make recommendations for care plan adjustments.
Requirements:
- Active RN license in the state of application.
- Valid CPR certification.
- Medicare Skilled Nursing experience preferred.
- Basic understanding of Oasis.
- 1-year RN experience in a healthcare setting.
