Job Title: Community Health Navigator OHP

Join the Urban League of Portland as a Community Health Navigator OHP and play a vital role in connecting families with affordable health insurance options. As a key member of our team, you will provide outreach, education, and cultural mediation to African Americans in North, Northeast, and Outer East Portland, increasing awareness about the availability and eligibility for enrollment in various Health Plans.

Job Summary:

The Community Health Navigator will develop, implement, and evaluate an enrollment program in accordance with contractual goals under the guidance and leadership of the Programs Director. This role requires a strong understanding of community health, social work, or a related field, as well as excellent communication and interpersonal skills.

Key Responsibilities:
  1. Develop and implement an enrollment program to increase awareness and access to affordable health insurance options for African Americans in North, Northeast, and Outer East Portland.
  2. Create educational materials and conduct outreach to inform families of available health plans and services.
  3. Develop and maintain professional relationships with community partners, including schools, after-school programs, community clinics, local organizations, and faith-based communities.
  4. Assess clients' involvement in services, identify barriers to care, and ensure compliance with contract requirements.
  5. Collect and maintain accurate, complete, and current written and database records/files.
  6. Available to work occasional evenings and weekends as required to meet program goals.
Qualifications and Skills:

Any combination of education and experience that demonstrates the ability to perform the duties of the position is qualifying. This typically includes a Bachelor's Degree in Community Health, Social Work, or a related field, or a minimum of three years of experience in community outreach, health education, or social services, working with and relating to African Americans.

Preferred qualifications include Community Health Worker (or other traditional health worker) Certification, commitment to and knowledge of social justice issues, and the dynamics of social change, the history and mission of the Urban League of Portland.
