Job Summary

The RN Stroke Navigator will serve as the primary point of contact for patients before and after hospital discharge, providing coordination between the patient, primary care physician, neurologist, and other specialists. This role will perform early post-discharge screening for medication compliance, healthy lifestyle, depression, medical complications, and caregiver burden, while advocating for the patient's needs and anticipating their requirements.

Key Responsibilities
  • Coordinate patient care between healthcare providers
  • Perform post-discharge screening for medication compliance and other health-related issues
  • Advocate for patient needs and anticipate their requirements
  • Collaborate with healthcare teams to ensure seamless patient care
Requirements
  • Bachelor's Degree in Nursing or equivalent
  • Oregon Registered Nurse License upon hire
  • National Provider BLS - American Heart Association certification
  • 5 years of related neurological clinical experience
Preferred Qualifications
  • Master's Degree in Nursing
  • NIHSS Certification
  • Stroke Certified Registered Nurse certification
  • American Association of Neuroscience Nursing membership and certification
  • Experience working with patients with neurological conditions and their families
  • Experience working in an Ambulatory setting
