Job Title: Navigation Specialist

Job Summary:

The Navigation Specialist will be a key member of our team, working closely with Case Managers and participants to support housing search and placement. This role will also involve program coordination, including document assessment and management for the Rapid Rehousing program.

Key Responsibilities:

  • Develop and maintain relationships with community partners to identify opportunities for participants.
  • Assist with participant roster management, intake, exit, and follow-up documentation.
  • Maintain accurate and efficient physical and electronic records, including HMIS data entry and reporting.
  • Participate in community partner meetings and provide information on agency services and requirements.
  • Communicate effectively with team members, staff, and participants.
  • Perform other assigned tasks and duties as needed.

Requirements:

  • 1-2 years' experience in social services, preferably in a navigation or housing specialist role.
  • Knowledge of housing first and harm reduction models.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.

Preferred Qualifications:

  • Bachelor's degree in social work, psychology, or a related field.
  • Relevant certification or training.
  • Lived experience.

Work Environment:

This role requires working indoors and outdoors in various weather conditions. The ability to lift up to 50lbs and stand for extended periods is necessary.
