Job Summary:

At Star of Hope, we are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for managing the life cycle of our clients, including developing their short and long-term objectives, holding them accountable, and providing guidance and support to help them achieve their goals.

Key Responsibilities:

Conduct one-on-one assessments to identify appropriate programming for residents
Develop and implement individualized plans to help residents achieve their goals
Provide guidance and support to residents to help them overcome challenges and achieve success
Maintain accurate records on each resident
Enroll residents in appropriate programs and classes as needed
Provide referrals to residents for additional support and services

Requirements:

B.A. or B.S. in Social Science or related field
1 year of experience in social services or a related field
Excellent communication and interpersonal skills
Ability to work respectfully and professionally with clients and staff
