Case Manager

Job Summary: We are seeking a skilled Case Manager to join our team at Quality Talent Group. As a Case Manager, you will play a critical role in ensuring the seamless transition of patients through the healthcare system.
Key Responsibilities:
- Assess patients' needs and develop comprehensive care plans to ensure optimal outcomes.
- Collaborate with healthcare teams to coordinate patient care, including discharge planning and post-acute care arrangements.
- Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making.
- Monitor patients' progress and adjust care plans as needed to ensure timely and effective transitions.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Bachelor's degree in Nursing or related field.
- Three years of hospital nursing experience, with a focus on case management or a related field.
- Current RN licensure in Texas or Compact State licensure within 60 days.
- Strong assessment, organizational, and problem-solving skills.
- Ability to work independently and as part of a team.
