Job Summary: We are seeking a skilled Case Manager to join our team at Star of Hope. As a Case Manager, you will be responsible for managing the life cycle of our clients, including developing their short and long-term objectives, holding them accountable, and handling grievances and discipline. You will also be responsible for maintaining accurate records on each resident and ensuring they are following a program that will assist in their transition from homelessness to self-sufficiency.

Key Responsibilities:

  • Manage the life cycle of clients, including developing short and long-term objectives and holding them accountable
  • Maintain accurate records on each resident
  • Ensure residents are following a program that will assist in their transition from homelessness to self-sufficiency
  • Model Christian character and advise and counsel residents using Biblical principles
  • Conduct assessments and log pertinent information in the Case Worthy system
  • Consult with residents on a weekly basis and make detailed documentation for the file
  • Enroll residents in appropriate programs and classes within the Case Worthy system
  • Assist residents with social services, including obtaining identification and benefits
  • Provide shadowing supervision to new Case Manager staff
  • Participate in regular staffing meetings and refer residents to appropriate programs or counselors

Requirements:

  • B.A. or B.S. in Social Science or related work experience
  • 1 year working experience in the field of Social Science as a Case Manager
  • Good computer skills and knowledge of Microsoft Windows, Microsoft Word, and Excel
  • Valid Texas Driver's License with acceptable driving record and current liability insurance
  • Average to above aptitude in counseling and conflict resolution

Work Environment: Indoor office environment. Schedule may be subject to change to meet the business needs of the mission. Local travel may be required. Exposure to infectious diseases, poor hygiene, drug usage, verbal and physical abuse, and other emergency situations relative to the homeless population is normal.
