Hospital Social Worker

We are seeking a highly skilled and compassionate Hospital Social Worker to join our team at MultiCare Health System. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and support to our patients and their families.
Key Responsibilities- Provide Cross-Continuum Care Coordination: Work closely with healthcare teams to ensure seamless transitions of care, from hospital to home or other care settings.
- Conduct Psychosocial Assessments: Assess patients' social and emotional needs, identifying opportunities for improvement and developing personalized care plans.
- Develop and Implement Care Plans: Collaborate with patients, families, and healthcare teams to create and implement comprehensive care plans that address patients' physical, emotional, and social needs.
- Provide Education and Support: Educate patients and families on disease management, treatment options, and available resources, providing emotional support and guidance throughout the care process.
- Collaborate with Healthcare Teams: Work closely with physicians, nurses, and other healthcare professionals to ensure comprehensive and coordinated care.
- Master's Degree in Social Work: Possess a Master's degree in Social Work from an accredited program.
- Licensed Social Worker: Be licensed as a Social Worker in the state of Washington, with a commitment to obtaining LICSW within four years.
- Experience in Healthcare: Possess a minimum of three years of experience in healthcare, preferably in a hospital or healthcare setting.
- Strong Communication Skills: Demonstrate excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Leadership Experience: Possess leadership experience, with the ability to lead and manage teams, and make decisions in a fast-paced healthcare environment.
