Job Summary:
MEDSTAR HEALTH is seeking a highly skilled Clinical Social Worker to join our team at MedStar Georgetown University Hospital. As a Clinical Social Worker, you will play a vital role in providing social work services to patients and families, collaborating with healthcare professionals, and facilitating communication regarding psychosocial issues.
Key Responsibilities:
- Provide clinical social work services to identified patients and families, including assessment, diagnosis, and treatment planning.
- Collaborate with other healthcare professionals as part of a multi-disciplinary team to ensure comprehensive care.
- Facilitate communication regarding relevant psychosocial issues of patients and families to healthcare teams through recording in medical records, consultation, and participation in relevant meetings.
- Function as a coordinating link between patient, family, hospital, and community resources to assure continuity of care.
- Represent social work perspective and serve as department representative to designated Hospital and Lombardi Cancer Center committees and community organizations.
- Participate in orientation, training, and education programs for students, physicians, nurses, and other healthcare professionals as required.
- Assume responsibility for identifying training needs, locating resources, and promoting own professional development.
- Collaborate with other disciplines in development of new programs that enhance the quality of services to the Oncology Patient/Lombardi Cancer Center patient and their significant others.
- Collaborate with Clinical Nurse Coordinator staff and assist Discharge Planning Case Management Staff to facilitate appropriate placement of patients.
- Actively cooperate as a participant in the Social Work Department, assuming backup-up and on-call responsibilities, committee work, research, QA/QI, and other duties as appropriate.
Requirements:
- Master's degree of Social Work from an accredited training program.
- 2 years' experience with acute and chronically ill patients (Hospital setting preferable).
