Provides direct patient care and coordinates activities of Cancer Institute Clinical Social Workers and interns. Assigns duties and organizes activities of social work staff in providing counseling and case management services. Provides consultation services and direct clinical interventions to a number of specialty services. Oversees routine operational tasks and the implementation of program development tasks.
Education
  • Master's degree in Social Work degree from a program accredited by the Council on Social Work Education required
Experience
  • 1-2 years of progressively more responsible job related experience required
Licenses and Certifications
  • LICSW - Licensed Independent Clinical Social Worker in the District of Columbia required
Assists staff through individual and group supervision in analyzing case problems and in improving their diagnostic skills. *Interviews patients regarding medical problems, physical and mental impairments, family history, finances, etc., to determine the nature and degree of difficulties and the most appropriate treatment plan. *Counsels patients individually or in groups on a planned basis and in emergencies. *Trains and supervises up to 5 graduate-level interns doing their practicum in the hospital. *Schedules and assigns work, ensuring adequate staff coverage.*Represents Clinical Social Work program in community or in interdepartmental activities.Compiles records and documents all assessments, recommendations and treatment planning activities for medical record.*Monitors and evaluates program activities of the team, ensuring quality assurance and up-to-date forms and documentation. *Prepares statistical reports on extent, nature, and value of services.*Develops care plan including identifying and recommending referrals to appropriate service providers and community-based organizations. Assists with facilitating access to services in an empowering way that promotes client control and learning. Participates in continuity of care between inpatient and outpatient to provide referral, support and liaison for patients and families. Assists patients by alleviating barriers to care.*Monitors day to day activities, ensures program forms are up-to-date, evaluates processes of and makes necessary changes based on this assessment and program needs.*Develops, evaluates and conducts outpatient intake screening and follow-up interviews with all referred and/or assigned client and/or family members at WCI or over the telephone to evaluate patients, family, and caregivers' psychosocial needs (medical, emotional, financial, environmental, etc). *Provides patient navigation services, by facilitating resources and processing applications for various patient assistance programs, including but not limited to financial, Medicaid enrollment, needy sick fund, free medication, transportation, social security benefits, disability benefits and insurance coverage. *Maintains clear, accurate, and up-to-date documentation (monthly statistical reports, assessments, care plans, progress reporting, contacts, referrals, productivity) on all outpatient clients in an ethical and confidential manner. Documents all activities associated with oversight of each patient and all services being provided. Must meet documentation requirement of the WCI and the District of Columbia. This includes but is not limited to completion of the comprehensive intake assessment; development of the comprehensive intervention plan utilizing interdisciplinary team members, client, family members and/or legal guardian; ensuring the proper implementation of services and all other activities to the efficient care of the patient. *Refers to interdisciplinary team at WCI as appropriate, including but not limited to, palliative care, nutrition, psychosocial counseling, spiritual care, home care, hospice, durable medical equipment, etc.Communicates and works in conjunction with key staff members such as nurse care coordinators, the palliative care team and community agencies to identify resources, coordinate care for patients and reduce duplication of services.*
