The ideal candidate will possess strong clinical work experience, exceptional communication skills, and the ability to work independently and collaboratively as part of a supportive team.

Key Responsibilities:
  • Facilitate care coordination to patients and their families
  • Organize access to tests, procedures, and results
  • Assist in the formation and implementation of discharge plans
  • Work collaboratively with other departments and agencies to ensure continuity of care
  • Make recommendations based on evidence and best practices
Requirements:
  • BSN, RN licensure
  • Strong understanding of Swanson's nursing model
  • Direct patient care experience
  • Exceptional communication skills
  • Ability to work independently and collaboratively
Preferred Qualifications:
  • A minimum of three years of experience providing patient care as a BSN, RN
