Position SummaryClinical pharmacy services in the outpatient setting are a key area of focus for the GW MFA. We are seeking a highly skilled Clinical Pharmacist to join our team and provide comprehensive pharmaceutical care and disease management in the ambulatory care setting to adult patients with cancer.
Key Responsibilities- Build a Comprehensive Pharmacy Clinic: Collaborate with physicians to see patients and bill for services, ensuring seamless care coordination.
- Medication Reconciliation: Perform thorough medication reconciliation for all patients seen at the clinic, ensuring accurate and safe medication management.
- Liaison and Coordination: Serve as a liaison between the Department of Pharmacy and Hematologic Malignancies Clinic to ensure all activities are coordinated and consistent.
- Patient Education: Educate patients and their families about their treatment plans and subsequent medication management, promoting informed decision-making.
- Medication Management: Responsible for all medication management goals set forth by the institution or Department of Pharmacy, ensuring adherence to established protocols.
- Toxicity Management: Manage toxicities caused by chemotherapy agents for all patients seen in the clinic, with a focus on toxicities caused by oral oncolytics.
- Order Verification: Review all chemotherapy orders written/placed by clinic physicians prior to being sent to pharmacy, ensuring accuracy and safety.
- Transitions of Care: Responsible for the transitions of care for patients, ensuring seamless care coordination.
- Coverage and Support: Provide coverage of the acute care services, including order verification, profile review, chemotherapy order preparation, discharge counseling, and clinical consultation when the acute care Clinical Specialist is absent.
- Policies and Procedures: Collaborate with the acute care pharmacist responsible for the Medical Oncology Hematology Service on policies, procedures, and guidelines associated with this patient population.
- Medication Use Evaluations: Perform medication use evaluations to identify areas for improvement and optimize patient care.
- Staff Education: Educate staff on guideline updates and practice advancements associated with this patient population, promoting ongoing learning and professional development.
- Referral Coordination: Refer patients seen in the clinic to GW's Outpatient Pharmacy for all outpatient medications, including oral oncolytics.
- Specialty Pharmacy Services: Identify and enroll eligible patients for specialty pharmacy services, ensuring timely access to necessary medications.
- Insurance and Financial Assistance: Coordinate with the Specialty Pharmacists and the Medication Assistance Program to verify patient insurance requirements and provide direction for available financial assistance resources.
- Education: A Doctor of Pharmacy degree from an accredited institution or an equivalent combination of training and experience.
- Licensure: Current or valid licensure as a Pharmacist in the District of Columbia or the eligibility to earn such licensure.
- Board Certification: Board Certification in Oncology Pharmacy or another BPS specialty.
