Mental Health Case Management Clinician Job Description

Job Summary: The County of San Diego is seeking a highly skilled Mental Health Case Management Clinician to provide adult mental health services and significantly improve the lives of individuals with mental illness.

Key Responsibilities:

  • Provide case management services for voluntary and involuntary clients diagnosed with mental illness.
  • Conduct behavioral health assessments to determine mental health and social service needs of clients.
  • Develop psychosocial rehabilitative service plans by collecting and evaluating client information and preparing written summaries.
  • Collaborate with clients' relatives, interested persons, and community agencies, and participate in interdisciplinary team meetings and interactions with families in care planning/long-term care.
  • Provide assistance to adult clients in problem-solving, counseling, placement services, daily living, social and self-sufficiency activities, managing money, and obtaining eligibility for benefits.

Requirements:

  • An Associates of Arts degree from an accredited U.S. college or university, or a certified foreign studies equivalency in social work, psychology, counseling, or a related field, and six (6) years of experience in a mental health setting providing case management services to individuals with serious mental illness.
  • A bachelor's degree from an accredited U.S. college or university, or a certified foreign studies equivalency in social work, psychology, counseling, or a related field, and four (4) years of experience in a mental health setting providing case management services to individuals with serious mental illness.
  • A master's degree from an accredited U.S. college or university, or a certified foreign studies equivalency in social work, psychology, counseling, or a related field, and two (2) years of experience in a mental health setting providing case management services to individuals with serious mental illness.

Preferred Qualifications:

  • Licensed Clinical Social Worker (LCSW), Marriage and Family Therapist (MFT), Licensed Professional Clinical Counselor (LPCC), or an equivalent license issued by the State of California or other recognized national or state agency or organization.
  • Certification as a Rehabilitation Counselor from a recognized national or state agency or organization.

Working Conditions: The ideal candidate will be able to work independently in field work situations and have experience providing case management services to adult clients who have serious mental illness.
