About the Job

We are seeking a highly skilled and compassionate Case Manager to join our team at Telecare Corporation. As a Case Manager, you will play a vital role in supporting individuals with serious mental illness to live independently in their chosen community.

Job Summary

Under the direct supervision of a senior Case Manager, you will serve as a primary support and case manager for individuals with mental illness and other co-occurring disorders. Your primary responsibility will be to assist these individuals in achieving their goals and living as independently as possible.

Key Responsibilities
  • Provide direct support and case management services to individuals with serious mental illness
  • Assist individuals in developing and achieving their goals, including obtaining employment, education, and housing
  • Coordinate with other service providers, including healthcare professionals, social workers, and law enforcement
  • Provide crisis intervention and support as needed
  • Document all interactions and services provided in a timely and accurate manner
Requirements
  • Bachelor's degree in Social Services or Human Services and (1) year of experience or High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Willingness to use the company vehicle to drive clients to appointments and groups, etc.
  • Willingness to use your personal vehicle to attend meetings, etc.
