Job Description

Job Summary

As a Care Coordinator at Alianza Wellness Center, you will play a vital role in helping individuals achieve their mental wellness goals. You will be responsible for providing case management services, including crisis intervention, discharge planning, and group and individual work. You will also provide mental health and substance use disorder counseling, screen clients for basic needs and symptom management, and facilitate placement as needed.

Responsibilities

  • Provide comprehensive case management services, including crisis intervention and discharge planning.
  • Develop and implement individualized treatment plans to support clients' mental wellness goals.
  • Facilitate group and individual work, including therapy sessions and counseling.
  • Collaborate with physical health providers to ensure whole-person care.
  • Advocate for clients' best interests and provide support in meeting their basic needs and symptom management.
  • Complete accurate and timely documentation, meeting minimum billable requirements for direct client services.
  • Facilitate placement, including pre-placement visits and transportation as needed.
  • Provide field-based services as required by program and client need.
  • Maintain confidentiality of clients' care.
  • Participate in scheduled program meetings.
  • Perform other relevant duties as assigned.

Qualifications

To be successful in this role, you will need:

  • A Bachelor's degree in a behavioral science field, such as psychology, social work, or counseling.
  • At least four years of experience providing services to individuals with mental illness.
  • Three years of experience providing case management to individuals with mental illness and/or substance use disorders is preferred.
  • A valid California driver's license and current auto insurance per contract requirements.
  • Cultural competency understanding and ability to work with diverse populations.
  • Bilingual skills are preferred.
  • Knowledge of Microsoft Office and EHR software is preferred.

Physical Requirements

This role requires:

  • Seeing, hearing, and speaking.
  • Stooping, bending, and moving around the facility.
  • Moving between offices and clients.
  • Driving.
  • Lifting and carrying heavy items.
  • Pushing, pulling, and dragging items.
  • Standing for long periods.

Working Conditions

This role involves working in a fast-paced, dynamic environment with frequent interactions with clients, staff, and external partners. You will be required to work in a variety of settings, including offices, community locations, and clients' homes. You will also be required to work flexible hours, including evenings and weekends, as needed.
