Telecare Corporation is seeking a dedicated Case Manager II to join our team. As a primary support and case manager, you will work with individuals with mental illness and other co-occurring disorders to help them live independently in their chosen community.

Key Responsibilities:

  • Under direct supervision, serve as a primary support and case manager for individuals with mental illness and other co-occurring disorders.
  • Assist individuals to live as independently as possible in their chosen community.
  • Develop and implement individualized service plans to meet the unique needs of each client.
  • Provide education and support to clients and their families to promote recovery and wellness.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support.

Requirements:

  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services.
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups.
