As a Case Manager II, you'll play a vital role in supporting individuals with mental illness and other co-occurring disorders to live independently in their chosen community.

Key Responsibilities:
  • Provide primary support and case management services to individuals with mental health needs
  • Assist clients in achieving their goals and living independently
  • Collaborate with healthcare professionals and community resources to ensure comprehensive care
  • Develop and implement individualized service plans to meet client needs

Requirements:
  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups
