Job Summary

We are seeking a skilled Housing Case Manager to join our team at Mental Health Systems. In this role, you will work closely with community organizations, government agencies, and property owners to secure living arrangements for our clients. Your primary responsibility will be to manage lease agreements for short-term, transitional, and permanent housing, ensuring that our clients' needs are met.

Responsibilities

  • Conduct weekly housing inspections, report any issues, and work with clients to resolve them.
  • Provide case consultation, crisis intervention, discharge placement, and targeted case management services to support clients in meeting their basic and housing needs.
  • Document client interactions and services provided in a timely and accurate manner, meeting contract requirements.
  • Locate housing opportunities to meet short-term, transitional, and permanent housing needs, and manage lease agreements.
  • Prepare subsidy applications, maintain client housing files, and ensure accurate rent calculations.
  • Assist with monthly tenant rent collection, transport clients to housing locations, and help them obtain necessary documents for housing placement.
  • Conduct housing screenings and assessments using the coordinated entry system.
  • Participate in shift coverage, including on-call duties, and provide field-based services as required by program and client needs.
  • Maintain confidentiality of client information and participate in scheduled program meetings.
  • Perform other relevant duties as assigned.

Qualifications

  • Bachelor's degree and 1 year of experience, or associate degree and 3 years of experience, or 5 years of relevant experience.
  • High School Diploma or GED required.
  • Valid California driver's license and current auto insurance, as per contract requirements.
  • Knowledge of housing needs for individuals with behavioral health needs preferred.
  • Cultural competency understanding required.
  • Bilingual preferred.
  • Knowledge of Microsoft Office and EHR software preferred.
