Healthcare Case Manager

The Home Health RN is responsible for providing quality care to patients in their homes. As a Healthcare Case Manager, you will be accountable for performing interdisciplinary case management and delivering age-specific therapy services.
Key Responsibilities:
- Conduct comprehensive assessments and evaluations to identify patient needs.
- Develop and implement personalized Plans of Care based on assessment findings and physician orders.
- Ensure compliance with state and federal regulations, as well as JCAHO standards, regarding the delivery of home care services.
- Provide case management for all disciplines and supervise Home Health Aides and Licensed Practical Nurses.
- Evaluate patient outcomes, communicate progress, and inform physicians of changes in condition.
Requirements:
- Graduate of an accredited school of Professional Nursing.
- Minimum 1-year acute care experience as an RN, with home care experience preferred.
- IV Therapy and phlebotomy experience preferred.
- Criminal History Screening.
- Computer skills.
- Current Florida Drivers License, automobile insurance, and reliable transportation.
- Must hold current AHA BLS Healthcare Provider Completion Card prior to start date and maintain.
- Current licensure in the State of Florida as a Registered Nurse.
Salary: $65,000 - $85,000 per year.
