Job Summary

We are seeking a skilled and compassionate Healthcare Specialist to join our team in delivering high-quality home health services.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' needs, including home safety, pain management, nutrition, fall risk, and DME requirements.
  • Develop and implement individualized Plans of Care based on assessment findings and physician orders.
  • Ensure compliance with state and federal regulations, as well as JCAHO standards, for the delivery of home care services.
  • Provide case management services for all disciplines, including supervision of Home Health Aides and Licensed Practical Nurses.
Requirements:
  • Graduate of an accredited school of Professional Nursing.
  • Minimum of 1 year acute care experience as an RN, with home care experience preferred.
  • IV therapy and phlebotomy experience also preferred.
  • Criminal History Screening.
  • Computer skills.
  • Current Florida Driver's License, automobile insurance, and reliable transportation.
  • AHA BLS Healthcare Provider Completion Card.
  • Current licensure in the State of Florida as a Registered Nurse.

Estimated Salary: $65,000 - $85,000 per annum, depending on qualifications and experience. We offer a competitive benefits package, opportunities for career growth, and a supportive work environment.
