Job Description:
We are seeking a highly skilled and compassionate registered nurse (RN) to join our team as a case manager. The ideal candidate will have one year of clinical experience as a Registered Nurse (RN) and be licensed in the state(s) they are assigned to work.

  • Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
  • Makes referrals to other disciplines as indicated by the patient's identified needs or documents rationale for not doing so.
  • Promotes patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques.

Responsibilities:
As a case manager, you will perform on-call responsibilities and provide staff education. You will also participate in clinical development and continuing education programs.

Required Skills and Qualifications:
- One year of clinical experience as a Registered Nurse (RN)
- Current RN license, specific to the state(s) you are assigned to work
- Current CPR certification
- Valid driver's license, reliable transportation and liability insurance
