Hospice & Palliative Care of Greensboro is actively seeking a Care Navigator to join our Integrated Health Services team, focusing on community-based support. This role is designed to enhance the well-being of individuals in our service areas.

Position Overview:
Employment Type: Full-time, Monday - Friday, 8:00 AM - 5:00 PM, with occasional weekend community events as required.

The Integrated Health Care Navigator collaborates closely with the clinical manager and director to deliver exceptional community-based case management services to participants enrolled in our programs. The Care Navigator serves as the primary liaison for participants and their caregivers, facilitating connections to vital community resources.

Key Responsibilities:
- Foster strong relationships with participants and caregivers to ensure ongoing engagement in medical care and adherence to treatment plans.
- Maintain individualized care plans that prioritize the needs of each participant.
- Identify and address barriers to care by connecting participants with essential resources and navigating healthcare systems.
- Work in conjunction with the Care Team clinical members to address complex medical, practical, and behavioral issues.
- Conduct intake assessments and document progress in the electronic medical record (EMR) system.

Communication:
- Effectively communicate both verbally and in writing with the Integrated Health Team, community healthcare providers, and partner organizations.
- Assess participant needs within their home or care facility and coordinate appropriate resources.
- Document care outcomes accurately in the EMR system.

Qualifications:
Education and Experience:
  • Bachelor's degree in social work or a related human services field (psychology, sociology, human relations).
  • Proven ability to establish and maintain effective working relationships with participants, colleagues, and the public.
  • Familiarity with Microsoft Word, Excel, and HCHB is a plus.
  • Preferred experience working with older adults, caregivers, and individuals facing dementia-related challenges.
  • Current CPR certification is required.
  • Other: A valid state-issued driver's license is mandatory, along with automobile liability insurance as required by the agency. Personal transportation is necessary.
