Role Summary:

The Social Work Care Manager - IPCM is responsible for delivering comprehensive care coordination and support services to individuals and families navigating complex social and health-related issues.

This role involves collaboration with clients, their families, healthcare professionals, and community resources to effectively address their unique challenges and enhance their overall well-being.

Key responsibilities include conducting detailed assessments of clients' social, emotional, and environmental situations, formulating tailored care plans, and facilitating access to essential services and resources.

Advocating for clients' rights and promoting empowerment is a fundamental aspect of this position, ensuring that patients receive equitable and high-quality care.

Additionally, this role encompasses crisis intervention, counseling, and advocacy, while also emphasizing preventive measures and community education.

Essential qualities for success in this position include compassion, empathy, and strong communication skills, which are vital for building trusting relationships and assisting clients in achieving their health and social stability goals.


Qualifications:

Required Education:

Bachelor's degree in Social Work

Certifications:

BLS (Basic Life Support) certification at hire date, required, or within 90 days of hire

Preferred Experience:

Two years of experience in an acute care hospital environment

Preferred Certifications:

Certification in Case Management
