The Opportunity

We're seeking a highly skilled and compassionate Field Care Coordinator to join our High Risk team. As a Field Care Coordinator, you'll play a critical role in supporting our patients with care outside of the traditional clinic setting. You'll work closely with patients, families, and care teams to facilitate telehealth visits, conduct clinical assessments, and coordinate care with community resources.

Key Responsibilities:
  • Facilitate telehealth visits in patients' homes, using technology to ensure seamless communication with providers.
  • Conduct standard patient assessments and documentation to facilitate complete visits by treating providers.
  • Clinical duties may include providing best-in-class telehealth setup services, performing and assisting with extended scope duties, and coordinating care with patients, families, and outside care teams.
  • Connect patients to community resources related to housing, transportation, food, and activities of daily living, among other social and physical barriers to health.
  • Assist patients and families in navigating and using community services or other resources in support of their goals.
Requirements:
  • At least 1 year of experience working in home-based healthcare services or senior living care settings or Emergency Medical Technician (EMT) experience.
  • Certified Medical Assistant (MA) and Basic Life Support (BLS) preferred or ability to obtain certification within 6 months of hire required.
  • Strong written and verbal communication skills; Spanish speaking is required.
  • A proven track record of persisting through change, demonstrating a forward-thinking perspective when under pressure, and consistently stepping up to take action on challenges.
  • A proven ability to build rapport and trust with patients or customers, while delivering individualized, human-centered, customer-focused care.
