Behavioral Health Care Coordinator Opportunity

As a Behavioral Health Care Coordinator, you'll play a vital role in ensuring patients receive the best possible care. Your expertise will be instrumental in coordinating long-term care needs for patients in the local community, working closely with an elite and dynamic team to make a meaningful impact.

Key Responsibilities:
  • Assess and implement individualized care strategies to ensure patients receive the most appropriate level of care.
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Manage care plans throughout the continuum of care as a single point of contact.
  • Communicate with stakeholders to ensure quality coordinated care and services are provided expeditiously to all members.
  • Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
Requirements:
  • Licensed Social Worker (LMSW, LBSW, LCSW) or current unrestricted RN license, NP, PLP, LPA, or LPC license in the state of Texas.
  • 3+ years of experience working within the behavioral health setting.
  • Intermediate level of proficiency with Microsoft Word, Excel, and Outlook.
  • Reliable transportation and ability to travel in the assigned area to visit Medicaid members in their homes and/or other settings.
  • High-speed internet at residence.
Preferred Qualifications:
  • Bachelor's degree, preferably in behavioral health or a clinical field.
  • Medicaid Waiver experience.
  • Case Management and/or Care Coordination experience.
  • Behavioral health experience.
  • Field-based work experience.
  • Experience with electronic charting.
  • Background in managing populations with complex medical or behavioral needs.
  • Knowledge of community resources.
  • Bilingual in English and Spanish.
