Job Summary

We seek an experienced LPN with outstanding direct care skills, experience in mental health and nutrition. Our ideal candidate will be driven, with exceptional interpersonal skills to develop lasting relationships with our clients and team members.

Responsibilities:
  • Provide education and consultation on medical health, medication, and medical topics. Offer one-on-one training in specific areas of need.
  • Develop and conduct groups and trainings on general health and wellness topics, including nutrition, exercise, and smoking cessation. Assist with accessing community resources to support health and wellness efforts.
  • Oversee, guide, and assist clients in understanding their medical conditions. Meet with clients and staff to discuss medical diagnoses, causes, symptoms, complications, and treatment options.
  • Perform direct medical care as needed and ordered by a physician, including monitoring vitals, wound care, etc. Provide interventions specific to individualized treatment plans for medical/health and wellness goal areas.
  • Review medical documentation and provide medical care consultation and coordination as needed. Assist with coordinating overall medical care and ensure follow-up appointments are scheduled.
  • Participate in committees, including the Agency's risk committee and Health and Wellness committee, providing medical consultation and direction.
  • Offer side-by-side interventions focusing on education and skills training related to health and wellness issues. Assist the team and clients by providing ongoing assessment methods for symptoms and treatment effectiveness.
Compensation

The estimated annual salary for this position is $74,680.

Schedule

This role operates Monday through Friday, 8am-4pm.
