Job Overview:

As a Case Manager specializing in support for individuals experiencing homelessness, your role will involve engaging with underserved community members (adults aged 18 and older, including youth transitioning out of care and families facing serious mental health challenges). You will operate in various community settings where these individuals seek essential services, including food, temporary shelter, and health resources.

Key Responsibilities:

  • Serve as a primary responder to complex crises, addressing both psychological and environmental factors.
  • Adopt a holistic approach to ensure that care and discharge plans comprehensively address the physical, social, and emotional needs of clients.
  • Assist clients in accessing housing resources and support.
  • Conduct thorough assessments of client needs and identify relevant resources, including basic necessities, while facilitating access to these services.
  • Maintain detailed documentation of services provided and develop individualized care plans with clients.
  • Connect clients with community resources as necessary.

Qualifications:

  • Bachelor's degree in health, psychology, counseling, social work, education, or a related behavioral health field.
  • One to two years of experience working with homeless populations or individuals with significant mental health needs.
  • Demonstrated knowledge, skills, and experience necessary for effective case management.
  • Personal lived experience is preferred but not mandatory.
  • Bilingual proficiency in Spanish is preferred but not required.
  • A valid driver's license and eligibility for agency automobile insurance are required.
