Key Responsibilities
  • Assess patient needs and develop individualized care plans under the supervision of the RN.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Collaborate with the patient's healthcare team to ensure seamless care transitions and effective communication.
  • Participate in ongoing education and training to maintain current knowledge and skills.
Requirements
  • Current LPN licensure in the State of Illinois.
  • 2 years of Med/Surgical clinical experience.
  • Certificate in Pharmacology and CPR certification.
  • Basic knowledge of computer use and ability to learn and use electronic health records.
