Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team in South East City. As an LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Responsibilities
  • Assure and improve the quality of care for patients and families, adhering to the Advocates MVP of Stewardship.
  • Facilitate patient and family education, promoting self-care and disease management.
  • Collaborate with the Registered Nurse (RN) to develop and implement individualized care plans.
  • Provide comprehensive care, including wound care, medication management, and vital sign monitoring.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in interdisciplinary case conferences and contribute to the development of care plans.
  • Document patient care and progress accurately and thoroughly, adhering to regulatory requirements.
Requirements
  • Current LPN licensure in the State of Illinois.
  • 2 years of Med/Surgical clinical experience, with a focus on home health care.
  • CPR certification.
  • Basic knowledge of computer use, including Microsoft Word and email access.
  • Ability to learn and use computer-based scheduling and documentation systems.
  • Excellent communication and documentation skills.
  • Good time management and self-organization skills.
Physical Requirements and Working Conditions

This role requires extensive travel within geographical boundaries, including all types of roads and weather conditions. You must be able to provide or arrange your own transportation to and from patients' homes and branch offices. Ability to work rotating weekends and holidays is required. Requires adherence to scheduled days and hours of work based on commitment to patient/client and work load assigned.

Compensation

We offer a competitive hourly rate of $24.35-$36.55, with opportunities for growth and professional development. Our comprehensive benefits package includes a sign-on bonus, competitive pay, referral bonus programs, tuition assistance, and mileage reimbursement.
