LPN - Home Health Care Specialist

Responsibilities:
  • Assess patient needs and develop personalized care plans to ensure optimal health outcomes.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Collaborate with healthcare professionals to coordinate care and ensure seamless transitions between settings.
  • Communicate effectively with patients, families, and caregivers to educate and empower them to manage their care.
  • Participate in quality improvement initiatives to enhance patient satisfaction and outcomes.
Requirements:
  • Current LPN licensure in the State of Illinois.
  • 2 years of Med/Surgical clinical experience.
  • Certificate in Pharmacology.
  • CPR certification.
  • Basic knowledge of computer use, including Microsoft Word and email access.
