At CenterWell, we're dedicated to providing exceptional home health care services to our patients. As a Home Health LPN, you'll play a vital role in delivering high-quality care and support to individuals in their own homes.

Key Responsibilities:
  • Provide skilled nursing interventions to patients in their homes, under the direction of a RN.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Confer with RN Supervisors to ensure seamless care coordination and communication.
  • Maintain accurate and detailed records of patient interactions and care provided.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support.
Requirements:
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
