Job Title: LPN - Home Health

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team in South East City. As an LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Job Summary:

The LPN will be responsible for delivering patient-centered care, conducting assessments, developing care plans, and implementing interventions to promote optimal health outcomes. You will work collaboratively with our interdisciplinary team to ensure seamless care coordination and communication.

Key Responsibilities:
  • Conduct thorough patient assessments to identify needs and develop individualized care plans
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
  • Collaborate with patients, families, and healthcare providers to ensure comprehensive care
  • Document patient progress and outcomes in a timely and accurate manner
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
  • Current LPN licensure in the State of Illinois
  • 2+ years of Med/Surg clinical experience
  • CPR certification
  • Basic knowledge of computer use, including Microsoft Word and email
  • Ability to learn and use computer-based scheduling and documentation systems
