Key Responsibilities- Assess and coordinate patient care plans in collaboration with the interdisciplinary team
- Provide expert nursing services to patients and their families, including education and support
- Develop and implement individualized care plans that meet the unique needs of each patient
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
- Document patient progress and outcomes in a timely and accurate manner
- Current Oregon RN license
- 1+ year of recent clinical experience as an RN
- Prefer certification as a Hospice and Palliative Care Nurse
- Must be fully vaccinated against COVID-19
- Current CPR certification
- Excellent clinical nursing skills, superior problem-solving ability, and commitment to continued learning and skill improvement
- Ability to work in a variety of patient settings, including care facilities, hospitals, and patient homes
- Ability to excel in high-stress situations and manage multiple priorities
- Computer competency and electronic medical recording keeping experience
- Ability to regularly walk, bend, stoop, kneel, push, pull, grasp, reach, handle, and lift up to 50 lbs unassisted
- Current driver's license, dependable transportation, and proof of current automobile insurance
