Job Summary

We are seeking a skilled and experienced Registered Nurse to join our team as a Hospice Case Manager. This is a full-time position that requires a strong commitment to providing exceptional care to our patients and their families.

Key Responsibilities
  • Manage a caseload of patients in the home health setting, providing comprehensive care and support to meet their physical, emotional, and spiritual needs.
  • Conduct thorough assessments of patients' conditions, develop and implement individualized care plans, and coordinate with other healthcare professionals to ensure seamless care delivery.
  • Collaborate with patients, families, and caregivers to educate them on disease management, treatment options, and end-of-life care.
  • Participate in on-call rotations to provide 24/7 support to patients and families.
  • Document patient interactions, progress, and outcomes in a timely and accurate manner.
Requirements
  • Current RN licensure in the state
  • At least 6 months of licensed experience in any setting
  • Valid Driver's license and auto insurance
  • CPR certification
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
