Job Summary

The St. Croix Hospice is seeking a highly skilled and compassionate Nurse Practitioner to join our interdisciplinary team. As a Hospice Care Specialist, you will provide comprehensive medical care to patients with terminal illnesses, working closely with our team of healthcare professionals to ensure the highest level of care and comfort for our patients and their families.

Key Responsibilities

  • Provide medical care and management of terminal illnesses, including palliative and supportive care, in compliance with state and federal regulations.
  • Conduct thorough assessments of patients' medical conditions, develop individualized plans of care, and coordinate with the interdisciplinary team to ensure seamless care delivery.
  • Collaborate with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
  • Participate in hospice interdisciplinary group meetings, contributing to the development and implementation of patient care plans.
  • Maintain accurate and comprehensive medical records, ensuring compliance with regulatory requirements.
  • Stay up-to-date with the latest medical research, best practices, and regulatory changes, applying this knowledge to improve patient care and outcomes.

Requirements

  • Master's degree in Nursing with a focus on Nurse Practitioner education.
  • Current nursing licensure and certification as a Nurse Practitioner.
  • Minimum of one year of experience in hospice or palliative care, with a strong understanding of the principles and practices of hospice care.
  • Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a team environment.
  • Ability to maintain confidentiality and handle sensitive patient information with discretion.
