Key Responsibilities
- Provide medical care and management of terminal illnesses, including palliative and supportive care, in compliance with state and federal regulations.
- Conduct thorough assessments of patients' medical conditions, develop individualized plans of care, and coordinate with the interdisciplinary team to ensure seamless care delivery.
- Collaborate with patients, families, and healthcare providers to ensure informed decision-making and optimal care outcomes.
- Participate in hospice interdisciplinary group meetings, contributing to the development and implementation of patient care plans.
- Maintain accurate and comprehensive medical records, ensuring compliance with regulatory requirements.

Requirements
- Master's degree in Nursing with a focus on Nurse Practitioner education.
- Current nursing licensure and certification as a Nurse Practitioner.
- Minimum of one year of experience in hospice or palliative care, with a strong understanding of the principles and practices of hospice care.
- Excellent communication, interpersonal, and problem-solving skills, with the ability to work effectively in a team environment.
