Position Summary
We are seeking a full-time Hospice Licensed Practical Nurse to join our team, providing care primarily in the Salem area, with some flexibility for remote responsibilities. The LPN will play a crucial role in delivering compassionate and proficient nursing care to patients at the end of life across multiple settings, including home care and residential facilities. This position focuses on ensuring patient comfort, symptom management, and family support during difficult times.
Key Responsibilities
- Conduct thorough physical assessments and evaluations of patients.
- Assist in the administration of medications and provide education on treatment protocols as directed by physicians.
- Monitor and accurately document vital signs and relevant medical information.
- Support patients with daily living activities, such as bathing, dressing, and personal hygiene.
- Deliver specialized care for individuals with cognitive impairments in memory care environments.
- Perform airway management tasks, including suctioning and oxygen delivery.
- Provide catheter care and assist with urinary catheterization procedures.
- Educate patients and their families on health management techniques for chronic conditions.
Qualifications
- Active Licensed Practical Nurse (LPN) license in Oregon.
- Experience in nursing, particularly in hospice, home health, or end-of-life care.
- Familiarity with hospice and home health regulations and standards.
- Strong knowledge of anatomy, physiology, and medical terminology.
- Exceptional communication skills to engage effectively with patients and their families.
- Proficient interpersonal skills to collaborate with patients, families, and healthcare professionals.
- Excellent observational, documentation, and organizational abilities.
- Capability to work independently and as part of a multidisciplinary team.
- Experience working with veterans and their families is advantageous.
- Strong problem-solving skills to address patient needs and emergencies efficiently.
