Receive and review patient referrals from healthcare providers, hospitals, and other sources.

Care Coordination:

Collaborate with clinical staff to develop individualized care plans for patients based on assessment findings and physician orders.

Coordinate patient schedules, including visits from field nurses, therapists, and other healthcare professionals.

Documentation and Record Keeping:

Ensure compliance with all federal, state, and local regulations governing home health services.

Participate in quality assurance activities, including chart audits and performance improvement initiatives.

Identify areas for process improvement and recommend changes to enhance efficiency and patient satisfaction.

Education and Support:

Provide education and support to patients and caregivers regarding their care plans, medications, and self-management techniques.

Serve as a resource for patients and families, answering questions and addressing concerns as they arise.

Qualifications:

Current licensure as a Registered Nurse (RN) in the state of practice.

Minimum of 2 years of clinical experience, preferably in home health or a related field.

Strong organizational skills and attention to detail.

Excellent communication and interpersonal skills.

Proficiency with electronic health record (EHR) systems and other software applications.

Ability to work independently and as part of a multidisciplinary team.
