Job Overview

The role of Community Support Specialist entails engaging directly with individuals during program activities, both within their residences and in the broader community. This specialist plays a crucial role in facilitating skill enhancement by executing tailored and general programs. Effective communication and collaboration with individuals and team members are essential, alongside a clear understanding and execution of service plans and Staff Action Plans as directed. Timely and accurate documentation is also a key responsibility.

KEY RESPONSIBILITIES

  • Engage in the individual's daily life at home to foster a person-centered atmosphere that promotes growth and independence in essential living skills. Provide customized support that honors the individual's preferences, cultural background, and strengths while ensuring their health, safety, and emotional well-being.
  • Assist in the development and execution of program plans.
  • Maintain accurate daily and bi-weekly progress reports, datasheets, timesheets, and other required documentation. Record each training session and communicate progress and challenges encountered with each client.
  • Participate in mandatory staff training and meetings, ensuring the protection of individual rights.
  • Notify Program Supervisors, individuals, and families of any changes in schedules, delays, absences, and other pertinent issues.
  • Provide verbal and written reports to Program Supervisors as necessary.
  • Complete required documentation electronically and confirm visits as needed.
  • Follow established policies and procedures regarding behavior management, emergencies, and incident reporting.
  • Accompany and assist individuals in community settings.
  • Exemplify appropriate behavior and offer support in accordance with Individual Service Plans.
  • Meet with assigned individuals multiple times per week based on coordinated hours.
  • Deliver personal care in alignment with the habilitation plan.
  • Support the mission and vision of the organization.
  • Perform additional duties as assigned.

QUALIFICATIONS

  • High School Diploma or GED.
  • Prior personal experience with individuals with developmental disabilities.
  • Access to a personal computer for documentation purposes.
  • Unrestricted NYS Driver's License (Preferred) and car insurance if applicable.
  • CPR Certification (Preferred).

PHYSICAL DEMANDS & WORKING CONDITIONS

  • Ability to utilize necessary technology for job functions.
  • Ability to lift up to 25 pounds. Must be capable of prioritizing tasks and managing time effectively. May require working non-standard hours.
  • May face stress-related situations and physically demanding conditions. The working environment includes family homes and community settings.

Job Type: Part-time

Pay: From $18.00 per hour

Schedule:

  • 10 hour shift
  • 12 hour shift
  • 8 hour shift
  • Day shift
  • Monday to Friday
  • Weekends as needed

Experience:

  • Caregiving: 1 year (Preferred)
