Care Coach Connect RN Case Manager Job Summary

We are seeking a skilled RN Case Manager to join our Care Coach Connect team in Texas. As a RN Case Manager, you will be responsible for coordinating care and providing ongoing nursing assessments of patients and their families. You will work closely with interdisciplinary team members and the Care Coach Connect supervising clinician to develop and implement individualized plans of care.

Key Responsibilities:

  • Conduct medical home visits on established Care Coach Connect patients, performing hands-on exams at each visit.
  • Conduct urgent/acute visits on established Care Coach Connect patients to keep them out of the hospital and receive the best medical care at home.
  • Provide ongoing assessments of the impact of life-limiting and/or chronic illness(es) on patients' physical, functional, psychosocial, and environmental needs.
  • Assess for caregiver burnout and provide community resources when appropriate.
  • Implement the individualized plan of care and recommend revisions as necessary.
  • Perform procedures, including wound care and dressing changes, phlebotomy, and injections (B12, vaccines).
  • Conduct advanced care discussions with patients and their families, documenting their wishes in the electronic medical record.
  • Initiate preventive and rehabilitative nursing procedures when appropriate.
  • Ensure continual assessment of patient and family needs from admission to Care Coach Connect throughout the course of care.
  • Provide ongoing evaluation of patient and family/caregiver response to care, recommending alterations to the plan when necessary.
  • Attend interdisciplinary meetings and serve as a crucial and vocal member of the team.
  • Solve problems by gathering and/or reviewing facts and selecting the best solution from identified alternatives.
  • Conduct telephonic nursing follow-up and case management when necessary.
  • Serve as a resource or consultant for LVN/MA.
  • Attend educational offerings to stay up-to-date on chronic medical conditions and comply with licensing requirements.
  • Establish trusting relationships with patients, caregivers, clinic staff members, and providers.
  • Collaborate with providers to recommend policies, procedures, and standards affecting Care Coach Connect patient care.
  • Exhibit professionalism and courtesy with all patients, physicians, and coworkers.
  • Inform providers of patient needs and outcomes of interventions as per standards.
  • Follow Care Coach Connect providers' orders regarding the scope and frequency of services needed based on acuity and patient/family needs.
  • Coordinate all patient/family services and prioritize needs with interdisciplinary team members.
  • Document progress toward established goals in the electronic health record as per standards.
  • Use the case management approach and refer to other services as needed.
  • Maintain a patient case load, daily visits, and point-of-care documentation levels as per standards.
  • Perform all other related nursing duties as assigned.

Requirements:

  • Registered Nurse with a current license to practice in the state of employment.
  • Current BLS certification or must obtain certification within 30 days of employment hire date.
  • 3+ years of experience in a physician's office, clinical, hospice, or hospital setting.
  • Demonstrate excellent nursing skills.
  • Proficient computer skills, including Microsoft Word, Excel, Access, and Outlook.
  • Demonstrate ability to interact productively with individuals and multidisciplinary teams.
  • Reliable transportation for daily travel to various locations as assigned, up to 80% of the time.
  • Valid driver's license within the state of work.

Preferred Qualifications:

  • Hospice and Palliative Care Nursing certification.
  • 2+ years of hospice experience.
  • Experience with home visits.
  • Experience related to advanced care planning and discussions with patients regarding end-of-life wishes.
  • Experience with community-based nursing.
  • Knowledge of palliative and hospice medicine.
  • Bilingual (English/Spanish) language proficiency.
  • Proven excellent verbal and written skills.
  • Proven excellent organizational and prioritization skills.

Physical & Mental Requirements:

  • Ability to lift up to 100 pounds.
  • Ability to push or pull heavy objects using up to pounds of force.
  • Ability to sit for extended periods of time.
  • Ability to stand for extended periods of time.
  • Ability to use fine motor skills to operate office equipment and/or machinery.
  • Ability to properly drive and operate a company vehicle.
  • Ability to receive and comprehend instructions verbally and/or in writing.
  • Ability to use logical reasoning for simple and complex problem-solving.
