Job Title: Registered Nurse (RN) Case Manager
Job Summary: AccentCare is seeking a Registered Nurse (RN) Case Manager to join our team in Lubbock, Texas. As a RN Case Manager, you will be responsible for providing high-quality patient care and case management services to our patients in their homes.
Responsibilities:
- Provide patient-centered care to patients in their homes
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with healthcare team members to ensure seamless care coordination
- Conduct regular patient assessments to monitor progress and adjust care plans as needed
- Communicate effectively with patients, families, and healthcare team members to ensure timely and effective care
Requirements:
- Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of Texas
- One (1) year experience as a RN
- Required Certifications and Licensures:
- Licensed to practice as a registered nurse in the state of Texas
- Must possess and maintain valid CPR certification while employed in a clinical role
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
