Job Summary

We are seeking a skilled LTSS Service Care Manager to join our team at Centene Corporation. As a key member of our care management team, you will play a critical role in developing and implementing holistic care plans for individuals with complex behavioral health needs.

Key Responsibilities
  • Develop and implement care plans that address the unique needs of each member, with a focus on behavioral health and wellness.
  • Conduct thorough assessments of member needs and develop strategies to support their goals and objectives.
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination and communication.
  • Provide education and support to members and their families on topics such as preventive care, procedures, and healthcare benefits.
  • Act as a liaison between members, healthcare providers, and community resources to ensure access to necessary services and support.
Requirements
  • Masters degree in Mental Health or Social Work, or equivalent experience.
  • 2-4 years of experience in case management, preferably in a behavioral health setting.
  • Licensed Behavioral Health Professional or RN with psychiatric experience.
  • Strong clinical documentation skills and ability to work independently with minimal supervision.
