Job Summary: We are seeking a highly skilled and compassionate Care Manager to join our team. As a Care Manager, you will be responsible for assessing and coordinating the care of complex and high-acuity populations, ensuring they receive the highest quality of care and support.
Key Responsibilities:
- Evaluate the service needs of members and develop personalized care plans to address their unique needs and goals.
- Coordinate and manage the care of members, working closely with healthcare providers, family members, and caregivers to ensure seamless care transitions.
- Monitor member status and progress towards care plan goals, making adjustments as needed to ensure optimal outcomes.
- Collaborate with the care management team to identify and address trends and areas for improvement in care delivery.
- Provide education and support to members and their families on disease processes, care options, and benefits available to them.
Requirements:
- Graduate from an accredited nursing program or bachelor's degree in a related field.
- 4-6 years of experience in care management or a related field.
- RN licensure and/or compact state licensure required.
