About the Opportunity:

Danbury Hospital is seeking a dedicated Care Coordinator Social Work LMSW to join our Care Coordination team. As a key member of our team, you will play a vital role in ensuring the highest level of patient care and satisfaction.

Responsibilities:

  • Develop and implement discharge plans, working collaboratively with patients, families, and healthcare teams;
  • Conduct thorough assessments of patient needs, identifying areas for improvement and opportunities for growth;
  • Collaborate with Clinical Care Coordinators to ensure seamless transitions between hospital and home or other care settings;
  • Provide counseling and support to patients and families navigating complex health issues, including substance abuse, financial need, and grief;
  • Assess patients for abuse, neglect, and domestic violence, reporting incidents to relevant authorities;
  • Lead patient and family conferences, offering education on Advanced Directives and Patient Rights;
  • Stay up-to-date with regulatory requirements and best practices in care coordination, ensuring compliance and continuous quality improvement;
  • Perform other duties as assigned by management.

Requirements:

To be considered for this opportunity, you must possess a Master's degree in Social Work (LMSW) and a minimum of three years' experience in counseling and discharge planning. A strong background in case management, prioritization, and problem-solving is essential. We prefer candidates with Case Management Certification.

Estimated Salary Range:

$32.64 - $65.01 per hour, based on experience and qualifications.
