**Salary Range:** $32.64 - $65.01 per hour

Job Description:

The Patient Advocate Specialist serves as a key member of the healthcare team, providing critical support to patients and families throughout the care continuum. This role requires a high level of emotional intelligence, empathy, and effective communication skills.

Key Responsibilities:

  • Collaborate with the Clinical Care Coordinator to identify patients' needs and develop comprehensive discharge plans
  • Provide education to patients and families on post-acute hospital care options, including advanced directives and patient rights
  • Coordinate applications for conservatorships and work with legal counsel and courts as needed
  • Participate in MDRs in NICU and provide grief counseling for fetal demise and sudden death
  • Ensure that patients receive necessary resources and referrals for social and financial support

Required Skills and Qualifications:

  • Bachelor's degree in Social Work or related field required; Master's degree preferred
  • Minimum three years counseling and discharge planning experience in an acute care hospital or equivalent
  • Case Management Certification preferred
  • Strong analytical and problem-solving skills
  • Ability to prioritize tasks and manage multiple projects simultaneously
